Adoption Application (Part I)

Thank you for your interest in adopting a pet from SFHA!  We are committed to finding permanent, responsible, and loving homes for our animals.

Please fill out both Part I and Part II of the Adoption Application completely.  Once your application is received, you will be contacted by one of our volunteers to discuss your responses.  A volunteer will review your application, contact your references, and do a veterinarian check.  A home visit will be performed, if necessary.

An adoption fee of $80 for cats and kittens or $125 for dogs and puppies will be collected at the time of adoption.  Discounts may be available for multiple adoptions.  This fee helps to cover the cost of caring for the pet including, but not limited to, food, routine and emergency veterinary care, spay/neuter surgery, vaccinations, medication, etc.

You will be contacted at the completion of the evaluation of your application.  If you are approved, arrangements will be made for you to pick up your new pet.

By submitting this application, you certify that all of the above information is true and recognize that any misrepresentation of facts may result in the loss of adoption priviledges.

You authorize verification of all statements in this application with the listed veterinarian, references and landlord.

By submitting, you are indicating that you have not been convicted of animal cruelty, neglect, or abandonment, and that agree to update your statement as changes occur pursuant to the Code of Virginia § 3.2-6546 D.

Keep in mind, a companion animal can live 15-20 years, so you must be prepared to make this commitment.  If at any time you are unable to care for a pet adopted through SFHA, you agree to contact and return that pet to SFHA.

SFHA reserves the right to refuse an adoption to anyone who, in its opinion, will not provide a suitable home for the pet.

Don't forget - our Adoption Application has two parts.  Please do not forget to complete Part II.
